Pesticide Recordkeeping for Private Applicators

For private applicators who use restricted use pesticides

Licensed private applicators are required by Federal law to keep records of any restricted use pesticides they apply. Applicators must keep these records for 2 years. You can use one of these forms (Version 1 in Word or PDF or; Version 2 in Word or PDF) or any other form as long as you keep these required elements:

  • Brand name of the pesticide
  • EPA Registration Number
  • Total amount of pesticide applied
  • Size of the area treated
  • Crop, commodity, stored product or site to which pesticide was applied
  • Location of treated area
  • Date of application
  • Certified applicator's name and license number

Certified applicators who purchase Restricted Use pesticides may be inspected by NMDA personnel for compliance with this law. For more details go to the USDA Pesticide Recordkeeping website or contact NMDA.
